Should you require a new or continued sick note please complete an online admin consultation between 8:00am - 18.30pm Monday to Friday. This will be responded to in three working days. The sick note will either be sent via email or text.
Please remember to include the following:
Your Name
DOB
NHS number (if you know it)
Date you want the sick certificate to start from
Period of time, how long for example 14 days
Reason for sick certificate request
Self Certification
If you are unwell, you can self certify for any illness for the first 7 days of that illness. Please use this link to download a form.
