New Patient Health Questionnaire for under 16's

The Practice area covers the Isle of Dogs and the area up to the New Providence Wharf development.

Please note that you must live within the practice boundary to be able to register with us. If you’re unsure, please look to our Location Map or visit NHS Choices (www.nhs.uk) to find your nearest GP or dentist.

In order to register a child under the age of 5, we will require their complete immunisation record (red book or equivalent). We will be unable to register them without this.
